Home based pre-school education for children with developmental delay, disabilities or any other special educational needs. Portage home visitors work in partnership with parents, helping parents to help their child through learning activities within the home. is a pre school home teaching service for children with additional needs. Staff work in partnership with the child, parents/carers and other professionals to support the child's development.

Age range - 0-3 unless child is not accessing Early Years provision.

What are the eligibility criteria to use the service?

• Home based pre-school education for children with developmental delay, disabilities or any other special educational needs. Portage home visitors work in partnership with parents, helping parents to help their child through learning activities within the home. Service - The child has to have a delay in more than one area of their development.

For children with more than 2 agency involvement supporting their development, they can access a key worker.

The Flexible Support Team who support Educating children with special educational needs, together with children who do not have special educational needs, in mainstream schools, wherever possible. Ensuring that children with special educational needs engage in the activities of the school together with the other children. into Early Years Setting,
Criteria: the child has been or will be going to Knowsley This involves building a picture of your child’s abilities, difficulties, behaviour, his/her special educational needs (if applicable) and the support required to meet those needs. A statutory assessment is a formal procedure which involves the collection of information from as many people as possible who have detailed knowledge about your child. and Placement Panel and it was agreed that the child has significant needs and that the child meets the criteria for the Education Health Care Plan

Universal: Level 1Children having their needs met through accessing universal services such as schools, health visiting, GP and so on. Plus - Level 2: Universal PlusTargeted response by single agency to support the additional needs.  Targeted services for children and families beginning to experience – or at risk of – difficulties, for example school counselling, parenting programmes, support for teenage parents and so on.

• Work borough wide with pre-school children (0-3 or until they access 3 year funding) and their families where there is a delay in more than one area of the child's development.
• Home based pre-school education for children with developmental delay, disabilities or any other special educational needs. Portage home visitors work in partnership with parents, helping parents to help their child through learning activities within the home. Home Visitors visit the family on  a weekly basis, establish a baseline and set activites each week to help them reach their full potential. 
• The team work in partnership with the parents/carer on skills they wish to teach their child.
• Early Support Key Workers - within the team there is a Designated key worker who works with 18 families where the child has involvement with 2 or more agencies..
• All Home based pre-school education for children with developmental delay, disabilities or any other special educational needs. Portage home visitors work in partnership with parents, helping parents to help their child through learning activities within the home. workers are trained as Early Support Key Workers and will key work for one family where they are the home visitor.
• Sibling support to families that are known to Home based pre-school education for children with developmental delay, disabilities or any other special educational needs. Portage home visitors work in partnership with parents, helping parents to help their child through learning activities within the home.
